What you send us

The inquiry form collects your name, email address, phone number, location if supplied, treatment interest, contact preferences, and any message you choose to include. It also includes the page where you made the request and campaign tags from that page, when present.

How the inquiry is handled

We use your inquiry to respond to your questions and arrange the next step you request. Requests are stored using Supabase. Contact details and inquiry information are also included in Telegram notifications to the site operator so the team can respond.

Keep medical information private

This is a general inquiry form, not a patient portal. Please do not include health records, diagnoses, medication lists, payment details, or other sensitive information. Discuss medical information directly with your treating provider through their appropriate intake process.

Contact permission

Submitting an inquiry asks us to contact you about that inquiry by your preferred method. The form does not sign you up for marketing messages or book treatment.

Questions about your information

Use the inquiry form to ask about information you previously sent, request a correction, or request deletion. State that your message is a privacy request and include only the contact details needed to identify the inquiry.
